Responsibilities Deliver high-acuity, evidence-based nursing care to LTAC patients Monitor, assess, and document complex patient conditions and responses to treatment Administer medications, manage IVs, vents, and tracheostomies safely Collaborate with physicians and interdisciplinary team on care plans and transitions of care Educate patients and families on treatment, recovery, and discharge plans Adhere to infection control, safety, and regulatory standards in all care activities Float and adapt to varying LTAC units and patient assignments as needed Maintain accurate, timely EMR documentation and critical communication Required Skills High-acuity LTAC nursing care Ventilator management Tracheostomy care Wound and drain care Medication administration and titration Central line and IV therapy Telemetry and continuous monitoring Care planning and coordination Patient and family education EMR documentation
