PRN LTAC RN

to deliver high-acuity, patient-centered care in a long-term acute care setting. You will manage complex, ventilator-dependent, and medically fragile patients, performing assessments, administering medications, and coordinating interdisciplinary care. Responsibilities Deliver high-quality nursing care to LTAC patients, including complex, long-term, and ventilator-dependent cases Conduct comprehensive patient assessments, monitor vitals, and document care accurately in EMR systems Administer medications, treatments, and wound care per provider orders and facility protocols Collaborate with physicians, therapists, and interdisciplinary team members to develop and update care plans Educate patients and families on care plans, discharge needs, and chronic condition management Respond promptly to changes in patient condition and participate in emergency response and codes Maintain compliance with infection control, safety standards, and regulatory requirements Float as needed within the LTAC setting and adapt to varying patient acuity and census Support a culture of respect, integrity, and patient-centered care aligned with Gifted Healthcare values Participate in ongoing clinical education, competency checks, and quality improvement initiatives Required Skills Long-term acute care (LTAC) nursing Ventilator management and tracheostomy care High-acuity patient assessment and monitoring IV therapy and central line management Wound care and complex dressing changes Medication administration and titration Electronic medical records (EMR) documentation Care planning and interdisciplinary coordination Infection control and patient safety practices Emergency response and code participation
