Key Responsibilities:

Conduct thorough annual risk assessments in patients homes Review and evaluate both current and prior medical and medication histories Educate patients on their health conditions and prescribed medications Identify care gaps, offering professional recommendations to enhance outcomes Complete health assessments without responsibility for on-site treatment planning or prescribing medications

Qualifications:

Valid Nurse Practitioner license in Tennessee Board Certification through AANP or ANCC Master of Science in Nursing from an accredited institution Current BLS/CPR certification At least 6 months of recent experience as a Nurse Practitioner preferred In this role, the average home assessment is anticipated to take roughly 45 minutes. Visits offer the chance to either perform annual health evaluations or post-discharge reviews, tailoring the approach to each individual members needs. Compensation for each evaluation ranges from $80 to $90, supplemented by a mileage stipend and administrative time, recognizing the importance of your time and expertise.
