Home Health/Hospice Springfield, TN Pay:
$50.00/hour
Schedule:
Monday-Friday, 8:00 AM to 4:30
PM Productivity:
30 points/week
Coverage Area:
Robertson, Sumner, and Cheatham Counties Mileage Reimbursement:
Yes (between patient visits)
On-Call:
Minimum 1 weekend per month with LPN backup Contract assignment: 3 months + (very likely contract to hire) Position Overview Our Home Health client is seeking an experienced RN to provide skilled nursing care to home health and hospice patients throughout the Springfield service area. This role is responsible for patient assessments, care planning, nursing interventions, documentation, education of patients and families, and collaboration with physicians and care teams to ensure quality outcomes. Required Qualifications Active Tennessee RN License Current BLS Certification Minimum 3 years of RN experience At least 1 year of Home Health RN experience Key Responsibilities Conduct patient assessments and develop/implement care plans Perform skilled nursing procedures and treatments Monitor patient conditions and report changes to physicians and leadership Complete accurate and timely clinical documentation Educate patients, families, and caregivers on health maintenance and disease management Participate in patient rehabilitation and ongoing care evaluations Ensure patient safety, comfort, and psychosocial needs are addressed Ideal Candidate Strong home health clinical background Excellent assessment and documentation skills Ability to work independently in the field Knowledge of the nursing process and care plan implementation Strong communication and patient education abilities Seeking an experienced Home Health RN with at least 1 year of recent home health experience. Monday-Friday schedule, mileage reimbursement, manageable productivity expectations, and coverage throughout Robertson, Sumner, and Cheatham Counties.
