Key Responsibilities:

Perform thorough annual home health risk assessments for Medicare Advantage members Collect and review medical history, current conditions, and medication usage Educate members about their conditions and medications, supporting understanding and adherence Identify and communicate care gaps, recommending actionable next steps

This role does not require creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Tennessee Current Basic Life Support (BLS/CPR) certification Board Certification via AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution At least six months of recent Nurse Practitioner experience is preferred This per diem position offers competitive compensation of $80$90 per evaluation, along with a mileage stipend and compensation for administrative tasks.
