Full Job Description Summary:
Serenity Hospice Solutions has a teams of Doctors, Nurses, Counselors, Social Workers, Chaplains and Volunteers that provide care for the patient and the whole family. Serenity utilizes a team-oriented approach to medical care, pain management, and emotional and spiritual support that is tailored to each patient's unique situation. We provide the care that our clients need at the times that they need it most.
Duties/Responsibilities:
This position would communicate with staff, physicians, referral sources and prospective clients regarding hospice services and appropriate admission criteria. Handle initial physical and emotional assessment of the patient/family upon admission and reviews the hospice concept with them. Develop a detailed, written POC for each patient admitted in conjunction with the IDT, patient, family and attending physician.
Qualifications:
Graduate of an accredited school of nursing with current Texas license as a Registered Nurse. Current certification in basic life support. Excellent organizational skills to handle a variety of tasks simultaneously. Effective interpersonal skills. Excellent communication skills, both oral and written. Ability to accommodate multi-faith beliefs and backgrounds of patients/families. Ability to function as part of interdisciplinary team. Ability to travel to multi-work locations during workday. Must have reliable, insured vehicle and valid Texas Driver's license.
