Job Overview Join our dynamic healthcare team as a Home Health Nurse, where your expertise will directly impact patient recovery and well-being in the comfort of their homes. In this vital role, you will deliver personalized nursing care, conduct comprehensive assessments, and coordinate with multidisciplinary teams to ensure optimal health outcomes. Your energy, compassion, and clinical skills will drive positive change in the lives of diverse patient populations, including those with complex medical needs and chronic conditions. Key Responsibilities Clinical Care & Assessment Perform comprehensive patient assessments, including vital signs, physical exams, and health history reviews to develop individualized care plans. Administer medications via injections, IV infusions, and other routes using strict aseptic and safety protocols. Provide advanced wound care, including suturing, ostomy care, and complex dressing changes. Care Coordination Collaborate with physicians and interdisciplinary teams to support diagnostic evaluations, treatment adjustments, and ongoing care planning. Document all patient interactions accurately and promptly in compliance with HIPAA standards. Patient & Family Education Teach patients and caregivers about disease management, medication use, nutrition, mobility aids, and self‑care strategies to promote independence and confidence. Experience & Qualifications Support patients who are ventilator‑dependent or require ongoing tracheostomy management in the home setting. Strong background in home health or long‑term care, including nursing homes or assisted living facilities. Hands‑on experience with critical care procedures such as ventilator management, trauma care, cardiac catheterization recovery, or infectious disease management. Skilled in caring for diverse populations across the lifespan, including pediatrics, geriatrics, and individuals with complex medical needs.
Pay:
$27.00 - $33.00 per hour
