Golden Villa Nursing Home 1.9 1.9 out of 5 stars 1104 South William Street, Atlanta, TX 75551 $15 an hour - Full-time Golden Villa 7 reviews $15 an hour - Full-time Golden Villa is seeking a compassionate, dedicated, and resident-focused Social Worker to join our interdisciplinary team. The Social Worker plays a vital role in promoting the emotional, psychosocial, and overall well-being of our residents while supporting families throughout the continuum of care. Responsibilities typically include psychosocial assessments, care planning, discharge planning, resident advocacy, family support, and coordination of community resources. Position Summary The Social Worker will assist residents and their families in addressing social, emotional, and adjustment needs related to long-term care, rehabilitation, and transitions of care. This position works closely with nursing, therapy, dietary, activities, and administration to ensure the highest quality of life for our residents. Essential Duties and Responsibilities Conduct psychosocial assessments and develop individualized care plans. Assist residents and families with adjustment to nursing home placement. Coordinate discharge planning and community resource referrals. Advocate for resident rights, dignity, and quality of life. Facilitate care plan meetings and family conferences. Provide counseling, crisis intervention, and emotional support as needed. Assist with advance directives and end-of-life planning discussions. Maintain accurate documentation in accordance with state and federal regulations. Participate in interdisciplinary team meetings and quality improvement initiatives. Support residents and families with Medicaid, Medicare, and other benefit-related concerns. Qualifications Experience in long-term care, skilled nursing, healthcare, or geriatric services preferred. Strong communication, organizational, and problem-solving skills. Ability to work effectively with residents, families, and interdisciplinary team members. Knowledge of federal and state nursing home regulations preferred.
