As a Field RN CM, you will coordinate care plans, assess patient needs, and collaborate with healthcare professionals to ensure optimal health outcomes while providing exceptional service and support to patients and their families. Responsibilities Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine care needs. Manage patients as a nurse case manager by developing personalized care plans that incorporate discharge planning, post-acute care, and outpatient services. Collaborate closely with healthcare professionals across hospital medicine, emergency medicine, ICU, PICU, Geriatrics, behavioral health, and home health settings to coordinate seamless patient transitions. Utilize Electronic Health Records (EHR) systems to document medical records accurately, track patient progress, and ensure compliance with HIPAA regulations. Perform diagnostic evaluations and monitor clinical case management activities to optimize utilization management and ensure appropriate resource allocation. Engage in patient service activities such as intake assessments, discharge planning, and client communication to promote understanding and adherence to treatment plans. Maintain up-to-date knowledge of health regulation policies, infectious disease care protocols, hospital experience standards, and medical terminology to deliver safe and compliant care. Qualifications Valid nursing license with current registration; experience in critical care environments strongly preferred. Proven background in hospital medicine, medical-surgical units, or outpatient clinics with acute care experience. Knowledge of infectious disease management, post-acute care processes, and inpatient/outpatient workflows. Strong understanding of physiology principles, diagnostic evaluation procedures, and medical records documentation standards. Excellent client communication skills combined with the ability to collaborate effectively with multidisciplinary healthcare teams. Proficiency in EMR/EHR systems and familiarity with HIPAA compliance requirements. Experience managing patients in diverse settings such as home health, or managed care environments is advantageous. Ability to perform comprehensive patient assessments including vital signs and clinical observations while demonstrating strong organizational skills for discharge planning and utilization management.
