Key Responsibilities:

Perform annual and post-discharge home health risk assessments to ensure patients' safety and continuity of care Review patient medical histories, current health conditions, and medication lists Educate patients on chronic disease management, medication adherence, and preventive health strategies Identify care gaps and recommend follow-up actions to improve clinical outcomes No prescribing or creation of treatment plans requiredfocus is on assessment, education, and recommendations

Qualifications:

Active Nurse Practitioner license in Texas Board Certification through AANP or ANCC Current CPR certification (BLS) Six months or more recent experience as a Nurse Practitioner preferred MSN from an accredited nursing program Earn between $80 and $90 per evaluation, with additional compensation for mileage and related administrative time.
