High Plains Senior Care Hospice Job Summary:
Primary functions are to provide nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency; and assumes the responsibility for coordination of care.
Responsibilities:
Promote hospice mission, goals and philosophy. Demonstrate commitment, professional growth and competency. Demonstrate compliance with policies, procedures, regulations defining scope of practice. Promote security and safety of Protected Health Information. Participate in programs and on committees as requested. Participate in ongoing education as required. Perform information visits, eligibility assessments and admissions as assigned. Assess patient/caregiver needs. Educate patient/caregiver as indicated. Provide services according to the Plan of Care. Report changes to the IDG. Coordinate with patient/IDG/Physician-develop/update plan of care to meet the needs. Attend IDG meetings. Provide supervisory visits for the Hospice Aide and Homemaker as applicable. Document services and submit timely. Perform on-call duties as assigned. Other duties as assigned
Qualifications:
Associate or Baccalaureate School of Nursing Texas State licensed as a Registered Nurse or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NCL) Two year's experience as a Registered Nurse in a clinical care setting preferred, hospice or home health preferred. Nursing skills as defined as generally accepted standards of practice. Strong communication and interpersonal skills; Sensitivity to issues of loss and grief; Ability to deal effectively with the demands of the job. Reliable transportation. State Driver's License. CPR certified; TB health screen or TB test/CXR according to policy; Hepatitis B Vaccination Consent/Declination
