Job Description

Position Overview We are seeking a skilled and dependable PRN Home Health Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes. This is a field-based role offering flexibility and autonomy, ideal for RNs who are comfortable working independently while collaborating with an interdisciplinary care team. Schedule & Flexibility Flexible scheduling based on your availability Visits are assigned as new patients are admitted Visit volume is based on patient assignments and generally increases as patient caseload grows. This role works well alongside other professional commitments Responsibilities Perform comprehensive patient assessments and reassessments Develop, implement, and update individualized plans of care Complete Start of Care (SOC), Resumption of Care (ROC), Recertifications, and Discharge visits as assigned Provide skilled nursing services including medication management, wound care, disease education, and treatments per physician orders Accurately complete clinical documentation in a timely manner Communicate changes in patient condition to physicians and the care team Ensure compliance with agency policies, state regulations, and Medicare guidelines Experience & Training Minimum of 1 year nursing experience (home health preferred) RNs without prior home health experience must complete: Agency training One unpaid shadow visit prior to independently taking patients This position is not supervised in the field ; RNs must be comfortable working independently Qualifications Active Registered Nurse (RN) license in the state of Texas Minimum of 1 year nursing experience (home health preferred) Reliable transportation and valid driver's license Strong clinical judgment, time management, and communication skills Ability to work independently in a field-based environment Compensation & Visit Structure This is a PRN, per-visit position Visits are typically scheduled for a minimum of 45 minutes , depending on patient needs and plan of care Compensation is based on visit type and complexity Why Work With Us Flexible, PRN scheduling Autonomous field work with office and clinical support Opportunity to build a caseload as referrals increase Supportive and organized care team
