Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan members Review and evaluate patients prior/current health status, ongoing medical conditions, and medication regimens Offer tailored education regarding medical diagnoses and prescribed medications Pinpoint potential care gaps and recommend follow-up actions for resolution Focus on assessment and education; no medication prescribing or treatment planning required

Desired Qualifications:

Active Nurse Practitioner license in Texas Board Certification through AANP or ANCC Current BLS/CPR certification Minimum of six months Nurse Practitioner experience preferred MSN from an accredited nursing program Nurse Practitioners are compensated $80-$90 per completed evaluation, in addition to a mileage stipend and payment for administrative time.
