In this per diem opportunity, you'll play a key role in promoting the well-being of Medicare Advantage plan members in Horseshoe Bay and throughout Llano County, conducting comprehensive visits in the comfort of patients homes. Each risk assessment typically lasts about 45 minutes and may include annual evaluations or post-discharge follow-ups, directly impacting the continuum of care for seniors and vulnerable individuals.

Key Responsibilities:

Perform annual risk assessments and health reviews for Medicare Advantage health plan members Assess and document prior/current medical history, chronic conditions, and medication profiles Educate patients on diagnoses, preventive health, and medication adherence Identify gaps in care and recommend appropriate interventions or referrals No prescribing, treatment plan development, or ongoing patient management required

Qualifications:

Nurse Practitioner (NP) licensure in the State of Texas Board Certification via AANP or

ANCC MSN

degree from an accredited nursing school Active BLS/CPR certification At least 6 months of current NP experience preferred You will earn $80 to $90 per completed evaluation, with a mileage stipend and additional compensation for administrative time.
