In this position, you will conduct home health risk assessments for Medicare Advantage plan members throughout Burnet County, focusing on locations such as Burnet and Marble Falls. Each assessment takes approximately 45 minutes and may involve annual health evaluations or post-discharge visits. Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage plan members Review prior and current medical history, chronic conditions, and medication usage Educate patients on their medical conditions and medication regimens Identify care gaps and provide actionable recommendations to patients and care teams No responsibilities for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in Texas Current CPR certification (BLS) Board certified as a Nurse Practitioner (AANP or ANCC) MSN from an accredited nursing program At least 6 months of recent NP experience (preferred) As part of this rewarding per diem opportunity, nurse practitioners will benefit from $80$90 per completed evaluation, with additional stipends provided for mileage and administrative duties.
