Job Description
Job Summary Join our dedicated home health team as a Licensed Vocational Nurse (LVN), on the day or weekend shift where your expertise will directly impact the quality of life for the patient within their own private residential home. In this vital role, you will provide compassionate, skilled nursing care tailored to the patient's individual needs, ensuring safety, comfort, and optimal health outcomes. Your energetic approach and critical thinking will help foster a positive environment for patients with diverse medical conditions that require long term care. This position offers an exciting opportunity to work independently while collaborating with a multidisciplinary team committed to excellence in home-based healthcare. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and administer medications according to personalized care plans. Provide wound care, tube feeding management, airway management which includes nebulizer treatments, CPAP Therapy, CPT Vest Therapy, Oxygen Therapy as needed, and suctioning support as needed for patient. Document all patient interactions accurately Coordinate with physicians, case managers, families, and caregivers to ensure seamless continuity of care across settings. Educate patients and their families on medication administration, disease management, nutrition, and safety measures to promote the client's well-being. Qualifications Valid LVN license in the state of Texas with current CPR certification. Recent graduate or limited Home Health Experience will be considered but proven experience working with individuals in specialized settings such as assisted living facilities or skilled nursing facilities is a plus.
