The ideal candidate will play a crucial role in coordinating patient care, ensuring that individuals receive the appropriate medical services and support throughout their healthcare journey. This position requires a strong understanding of various medical settings. Duties Assess and evaluate patients' needs to develop individualized care plans. Coordinate with healthcare providers, patients, and families to facilitate effective communication and care transitions. Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes. Provide education and support to patients regarding their health conditions and treatment options. Maintain accurate documentation of patient interactions, care plans, and progress notes. Collaborate with interdisciplinary teams in various settings Utilize knowledge of medical terminology and case management principles to enhance patient care delivery. Advocate for patients' needs within the healthcare system while ensuring compliance with policies and regulations. Qualifications Current Registered Nurse (RN) license in good standing. Experience in hospice care is preferred. Previous experience in case management is highly desirable. Strong understanding of medical terminology and healthcare systems. Excellent communication skills, both verbal and written. Ability to work collaboratively within a multidisciplinary team environment. Strong organizational skills with the ability to manage multiple priorities effectively.
