Job Description

Aveanna Healthcare is seeking a Private Duty RN to deliver compassionate, one-on-one skilled nursing care to pediatric and adult patients in their homes. In this role, you will perform comprehensive assessments, administer medications and treatments, manage complex clinical needs, and develop individualized care plans in collaboration with families and the care team. You'll document care accurately, uphold safety and infection control standards, and respond promptly to changes in patient status. Responsibilities Provide one-on-one skilled nursing care to pediatric and adult patients in their homes Assess, monitor, and document patient conditions and vital signs Administer medications, treatments, and therapies per physician orders Develop and update individualized care plans in collaboration with families and care teams Educate patients and caregivers on disease management, equipment, and home care safety Coordinate with physicians, therapists, and other clinicians to ensure continuity of care Maintain accurate clinical documentation and comply with regulatory standards Respond to changes in patient status and provide emergency interventions as needed Uphold infection control, safety, and quality standards in the home setting Participate in ongoing training and professional development offered by employer Required Skills Skilled nursing and clinical assessment IV therapy and medication administration Wound care and tracheostomy care Ventilator management Care planning and care coordination Electronic health record (EHR) documentation Patient and family education Infection control practices Emergency response and crisis management Time management and independent work in home settings
