Key Responsibilities:

Perform annual risk assessments in patients homes, typically lasting about 45 minutes per visit Review prior and current medical histories, examine medical conditions, and assess medication usage Educate individuals on their health conditions and medication regimens Identify gaps in care, making thoughtful recommendations for improvement

Qualifications:
