- What You'll Do
- Provide skilled nursing care in patients' homes
- Monitor patient conditions and report changes to the care team
- Administer medications and treatments as ordered
- Educate patients and families on care plans and disease management
- Complete accurate and timely documentation
- Collaborate with RNs, physicians, and interdisciplinary care teams
- Build trusted relationships with patients and caregivers
- What You'll Need
- Current LVN/LPN license in the state of practice
- Minimum of 1 year of nursing experience in acute care or similar setting
- CPR certification through American Heart Association or American Red Cross
- Strong communication and time management skills
- Compassionate, patient-focused mindset
- Reliable transportation, valid driver's license, and current auto insurance
