Position Responsibilities:

Coordinate and oversee all aspects of assigned patients' hospice care Develop and implement individualized plans of care with the interdisciplinary team Perform head-to-toe assessments, monitor symptoms, and manage medications Educate patients and families regarding condition, needs, and care Communicate patient progress with clinical leadership, physicians, and the care team Requirements Current Registered Nurse license.
