As a result, APC Home Health has become a leading provider of home health and Personal Assistance Services.
Job Summary:
Seeking RN on a PRN basis to support APC Home Health Service, Inc in the delivery of services to the VA community. Will be responsible in overseeing the provision of Personal Assistance Services to the VA Clients referred to the agency and oversee the education and training of the Home Health Aides assigned to the VA Clients to ensure competency in the delivery of all the Activities Of Daily Living (ADL's) and Independent Activities Of Daily Living (IADL's).
Job Responsibilities:
Case Manager- VA Clients Conduct starts of care home visits and visit documentation packets for the VA clients; Complete documentation visit packet of forms during home visit; Establish relationship and good rapport with Client and Clients family during home visit; Ensure Client gains complete understanding of the ISP-Service Plan authorized by the VA to include through review of the Supervisory Visits to be completed by an RN and PAS - Personal Assistance hours and tasks to de provided by the Home Health Aide as authorized by the VA; Able to effectively communicate Clients Service Plan (ISP) to the Attendant in the presence of the Client and Clients family; Able to communicate effectively to the Home Health Aide (in the presence of the Client), expectations in the delivery of tasks in a way that will meet the Client's needs and expectations of the services;
Supervision Home Health Aides:
Able to demonstrate and teach Home Health Aides how to perform the tasks as instructed in the Classroom training while ensuring the health and safety of the Client in the home; Able to evaluate the Home Health Aide with return demonstration to determine competency of all the Activities of Daily Living (ADL's) and Independent tasks of Daily Living (IADL's); Abel to determine when Home Health Aide is not able to demonstrate competency, provide additional education and training to further re-evaluate for competency; Ensure a Home Health aide is not allowed to perform a task where he/she has not been certified competent to perform independently without RN supervision; Able to ensure the Certified Home Health Aide will provide the authorized tasks in a safe manner. Requirements Current license (RN); Excellent communication skills; Experience in Home Health or Hospice setting preferred but not required; Experience supervising Nurses' Aides in Home Health or Hospice setting preferred; Experience teaching and mentoring Home Health Aides preferred but not required;
