Job Overview:
``` Lone Star Home Dialysis is seeking qualified, experienced and compassionate Home Hemodialysis Nurse ( LVN and RN) to provide one-on-one patient care ensuring the highest level of care, comfort, and safety.
Responsibilities:
Work with interdisciplinary team of physicians, director of nursing, social worker, dietician and BioMed Tech to develop individualized dialysis treatment plans, following the established plan for better care and outcome. Conduct comprehensive assessments of patients' health status and dialysis needs. Educate patients and their families on home dialysis procedures, equipment and self-care techniques. Perform hemodialysis procedures in patients' homes, adhering to established protocols and safety standards. Monitor patients' vital signs, fluid balance, and response to treatment during each session. Provide support and encouragement to patients ensuring treatment adherence and positive outcomes. Document all patient encounters, treatment plans, and outcomes accurately and in a timely manner.
Qualifications:
Current licensure as a Registered Nurse or Licensed Vocational Nurse A minimum of 18 months of acute or chronic hemodialysis dialysis experience (within the last 24 months) Current Cardiopulmonary resuscitation (CPR) certification Excellent assessment, critical thinking, and communication skills. Ability to work independently and as part of a multidisciplinary team. Strong commitment to patient-centered care, compassion, and empathy. Valid driver's license and reliable transportation.
