Job Description

Aveanna Healthcare is seeking a compassionate Private Duty Nurse LVN to provide one-on-one skilled care to pediatric and adult patients in their homes. In this role, you will administer medications and treatments, manage trachs, vents, and feeding tubes, and closely monitor patient conditions. You'll collaborate with an interdisciplinary team, support and educate families, and maintain detailed clinical documentation. Responsibilities Provide one-on-one skilled nursing care to pediatric and adult patients in the home setting Administer medications, treatments, and therapies per physician orders and care plans Monitor, assess, and document patient condition, reporting changes to the clinical team Operate and manage medical equipment such as vents, feeding tubes, and suction devices Educate and support patients and families on care routines and health management Collaborate with interdisciplinary team members to ensure coordinated, patient-first care Maintain accurate clinical documentation and comply with all policies and regulations Participate in ongoing training and competency reviews to support clinical excellence Required Skills Home health nursing Pediatric nursing Adult medical-surgical nursing Tracheostomy care Ventilator management Medication administration Patient assessment and monitoring Wound care Electronic medical records (EMR) documentation Infection control practices
