Our team is built on a foundation of care, professionalism, and integrity—and we're currently seeking an experienced and dedicated Field LVN to join our growing team.
Position Overview:
As a Field LVN, you will provide skilled nursing care to patients in their homes, working closely with physicians, case managers, and other healthcare professionals. This role requires a compassionate individual with strong clinical skills, a commitment to quality care, and the ability to work independently. You'll be responsible for delivering care in a variety of home settings and ensuring patients receive the highest level of care possible.
Key Responsibilities:
Patient Care:
Provide nursing care to patients in their homes, including assessments, wound care, medication administration, and monitoring of vital signs.
Care Coordination:
Work with the interdisciplinary team (physicians, RN case managers, physical therapists) to ensure optimal patient care and outcomes.
Documentation:
Maintain accurate and timely documentation in compliance with agency policies and regulatory requirements.
Patient Education:
Educate patients and their families on managing their health conditions, medications, and care plans.
Communication:
Regularly communicate with the clinical team and provide updates on patient status, progress, and any changes in condition.
Emergency Response:
Provide immediate care in the event of a medical emergency, stabilizing patients until further medical assistance is available.
Qualifications:
Education:
Must be a graduate of an accredited LVN program.
Licensure:
Valid LVN license in the state of Texas.
Experience:
Minimum of 1-2 years of nursing experience, preferably in home health or a related field.
Skills:
Strong clinical skills, excellent communication, and patient management abilities.
Certifications:
CPR and BLS certification required. IV certification and other specialized training a plus.
Driving:
Reliable transportation and a valid driver's license required, as the role involves travel to patient homes. Why Join Us?
