Divinity Healthcare is looking for a PRN RN Case Manager to join our family! Primary functions are to provide nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency; and resumes the responsibility for coordination of care.

EXPERIENCE AND QUALIFICATIONS

• Licensed as a professional in the state of Texas as a Registered Nurse
• Hospice or home health experience preferred

COMPETENCY/SKILL REQUIREMENTS

• Knowledgeable of Medicare, regulatory, and standards
• Excellent written and verbal communication skills
• Able to utilize a telephone for internal and external communications
• CPR certified
• Competent in care provision in area of professional licensure
• Strong computer skills - We currently use WellSky (formerly Kinnser) EMR
• Team player

JOB RESPONSIBILITIES

• Provides leadership and oversight of clinical activities to include, but not limited to, patient assessments, individualized care plan development, care coordination, on-site field visits, case management and case conferencing.
• Promotes hospice mission, goals and philosophy.
• Participate in care coordination to ensure proper communication between caregivers, patients, and physicians.
• Provide oversight of appropriate documentation, quality of care, contact with physicians, adherence to the care plan, and evidence of care coordination between disciplines.
• Delegate the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to Company, physician, and/or health facility procedures/policies.
