RN Case Manager Location:
Everman, TX, 76140 We are seeking a highly skilled and experienced Home Health
  • FT•RN Case Manager to join our team in Highland Park, TX.
The ideal candidate will have a strong background in home health care and be able to provide exceptional care to our patients.
Responsibilities:
Assess patient needs and develop individualized care plans Coordinate and manage patient care with other healthcare professionals Provide skilled nursing care to patients in their homes Educate patients and their families on proper care techniques and disease management Monitor patient progress and adjust care plans as needed Document patient care and progress in electronic medical records Communicate with physicians and other healthcare professionals regarding patient care Ensure compliance with all state and federal regulations
Requirements:
Current RN license in the state of Texas Minimum of 2 years of experience in home health care Strong clinical skills and knowledge of disease management Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong organizational and time management skills Experience with electronic medical records Valid driver's license and reliable transportation
