Key Responsibilities:

Conduct thorough annual and post-discharge home health risk assessments at patients residences Review and evaluate medical history, chronic conditions, and current medication regimens Educate members about their diagnoses and prescribed therapies during each visit Identify and document any potential gaps in care, offering actionable recommendations No requirement to create individualized treatment plans or prescribe medication

Qualifications:

Valid Nurse Practitioner license in Texas Current CPR (BLS) certification Board certification from AANP or ANCC Minimum of six months recent experience as a Nurse Practitioner preferred Master of Science in Nursing (MSN) from an accredited institution For each completed evaluation, compensation ranges from $80 to $90, with an additional mileage stipend and compensation for administrative activities.
