•Job Pay based on experience We are seeking a hospice case manager dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. Responsibilities Assess patients' medical needs and develop individualized care plans in collaboration with the healthcare team. Coordinate and manage patient services in hospice care in their own home. Maintain accurate medical records and documentation to ensure continuity of care. Provide education and support to patients and their families regarding treatment options and health management. Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes. Collaborate with interdisciplinary teams to facilitate effective communication among healthcare providers. Utilize knowledge of medical terminology, physiology, and emergency medicine to enhance patient care delivery. Advocate for patients' rights and needs within the healthcare system. Experience Current Registered Nurse (RN) license in good standing. Previous experience in case management or a related nursing field is preferred. Familiarity with medical records systems and documentation practices is essential. Experience in acute care settings, including ER or PICU, is highly desirable. Strong understanding of medical terminology and physiology is required. Excellent patient service skills with a focus on compassionate care delivery.
