Job Description
IMMEDIATE OPENING
- Full Time Registered Nurse Sunglo Home Health Services, Inc. has an immediate opening for a full-visit Registered Nurse (RN) - Rio Grande City and surrounding area. We are seeking Registered Nurse's to provide direct skilled patient care responsibilities include admissions, plan of care, providing treatments and working collaboratively with the interdisciplinary team to help meet positive patient care outcomes.RN DUTIES AND RESPONSIBILITIES
Registered Nurse RN Team Leader will do the initial evaluation for every new patient. The team member making the initial evaluation will have an adequate knowledge base and skills to provide individualized care to each patient, and for initiating the Plan of Care under the direction of the patient's physician. If the assigned RN is unable to see the patient, another team member will follow the patient under the direction of the RN Team Leader on a temporary basis. It is the responsibility of the RN Team Leader to assign an alternate nurse and to report any change to the RN Supervisor to the scheduler immediately. The RN Team Leader will be responsible for communicating with the physician and all other disciplines under his/her supervision in a timely manner. RN Supervisor to ensure appropriate team composition. Changes in the composition of the team may be each team will communicate on a daily basis as determined by the RN Supervisor and RN Team Leader to ensure conferences and will report to RN Supervisor daily and as necessary. RN will provide adequate coordination of team efforts and assignments and will schedule, lead and participate in case RN Team Leaders will be responsible for LVN and Home Health Aide supervisory visits. RN Team Leader will be responsible for submitting all clinical notes, completed admission forms, per visit sheets, recert documentation, and supply sheets within 24 hours. The RN Team Leader is responsible for being knowledgeable of Medicare reimbursement criteria, current charting requirements and dates of recertification of all patients under her/his care and supervision. RN Team Leader is responsible for reviewing all his/her documentation submitted for accuracy, appropriateness, and completeness in a timely manner as outlined by company policy. RN Team Leader will review team members' documentation as instructed by the RN Supervisor and as reported standards by Quality Assurance Department. RN Develops and implements discharge plans for Agency patients, doing discharge Verbal Order and Discharge Summary. RN will comply with agency policies and procedures. Promotes and maintains an agency environment that is in compliance with federal, state and local regulations. RN communicates information effectively to physicians, supervisors and others as needed in a timely manner. RN Manages the utilization of supplies/equipment to enhance productivity and maximize cost controls. RN Acts as a role model for other nursing personnel through exhibiting professional behavior toward patients, families, physicians, and other staff. RN will report Accidents/Incidents immediately to the DOPS. Will attend professional meetings and scheduled inservices as directed. Will rotate on-call with the other Agency RNs and complies with policy and procedures for weekly call.RN REQUIREMENTS
Bilingual (English/Spanish) At least 2 years RN experience with Medicare Understanding of Medicare Conditions of Participation's (COPS), Federal, State and Joint Commission guidelines and regulationsRN OASIS
(1 year) Knowledge with Kinnser/Wellsky (EMR)MUST HAVE COMPUTER SKILLS MUST HAVE PERSONAL COMPUTER RN QUALIFICATIONS
