Key Responsibilities:

Complete thorough annual and post-discharge home health risk assessments for Medicare Advantage participants Obtain and evaluate patients previous and current medical history, ongoing conditions, and medication lists Educate members regarding their medical issues and medication regimens in the comfort of their homes Identify potential care gaps and recommend appropriate next steps for patients and primary care teams Focus visits on risk assessmentno prescriptive authority or treatment plan formulation required

Qualifications:

Active Nurse Practitioner license in Texas Current CPR (BLS) certification MSN degree from an accredited nursing program Board certification via AANP or ANCC Minimum of six months of recent NP experience preferred As part of this per diem position, you will earn $80$90 per evaluation, with an additional mileage allowance and compensation for administrative time.
