Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review and evaluate patients prior and current medical histories, ongoing conditions, and medication records Educate patients about their medical conditions and appropriate medication usage Identify any existing gaps in care, offering recommendations to ensure optimal patient health outcomes Visits may include annual health assessments or post-discharge follow-ups Focus exclusively on evaluationno treatment plans or prescription authority required

Required Qualifications:

Current and unrestricted Nurse Practitioner License in Texas Active CPR certification (BLS) Board certification through AANP or ANCC Master of Science in Nursing (MSN) from an accredited nursing program At least 6 months of recent Nurse Practitioner experience preferred This role features compensation of $80$90 per evaluation, along with a mileage stipend and paid administrative time.
