Houston Hospice believes that the most valuable resource to an organization is it's Employees. Founded in 1980, we are the oldest, largest, independent non-profit hospice in the Greater Houston area. It is our mission to provide uncompromising, compassionate, end-of-life care to patients of all ages and their families. Serving 13 counties that surround the Houston area, we are committed to being the highest quality hospice provider wherever we serve. At Houston Hospice, we focus on caring, not curing. We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses.
Summary:
We are seeking a candidate who would be responsible for the personal care of patients/families on a field team as a participating member of the hospice team. Tasks are assigned by and performed under the supervision of a Registered Nurse.
Qualifications:
(one of the following) Satisfactory completion of a training and competency evaluation program which complies with state/federal regulations and one (1) year experience preferred. Be on the Texas Department of Human Services' Nurse Aide Registry with no findings against the Aide relating to client abuse or neglect or misappropriation of client property. (AND) According to Nurse Aid Registry, no evidence/findings against the Aide relating to client abuse or neglect or misappropriation of client property. No criminal history or employee misconduct record that would preclude hiring. Current certification in basic life support. Experience with terminal patients, oncology or home health services preferred. Ability to read, write and carryout instructions. Effective interpersonal skills. Understanding of Hospice philosophy of care. Ability to accommodate multi-faith beliefs and backgrounds of patients/families. Ability to travel to multi-work locations during workday and evenings as needed. Must have reliable transportation.
Duties/Responsibilities:
Assist with personal patient hygiene, elimination, feeding and mobility according to the patient's needs and plan of care. Assist with/provide environmental services that are essential to maintaining the patient and unit environment assigned. Document care provided and response to care according to documentation guidelines. Report changes in patient/family condition and/or needs to primary nurse. Complete twelve hours continuing education per calendar year to be prorated according to date of hire. Participate in weekend and holiday rotation. Assists with new employee training. Attend 100% of mandatory in-service meetings. Understand and adhere to organizational privacy practices and compliance programs. Assist in providing service that fosters the organization's mission, values, vision and goals. Perform other duties as required and/or requested by management. IND123
