As a Nurse Practitioner, you will complete comprehensive in-home evaluations for members residing in the areaserving communities such as Victoria and Bloomington. Each home visit assessment lasts approximately 45 minutes and may involve annual health check-ups or post-discharge evaluations. This position is designed for those who thrive on autonomy and value meaningful patient interactions.

Key Responsibilities:

Conduct thorough home health risk assessments for Medicare Advantage plan members Review medical history, current health conditions, and medication use Interview members about their health and overall wellness Provide education regarding diagnoses and prescriptions Identify gaps in healthcare and suggest actionable recommendations No need to develop treatment plans or prescribe medications

Qualifications:

Valid Nurse Practitioner license in Texas Active Board Certification (AANP or ANCC) Graduation from an accredited Master of Science in Nursing (MSN) program Current CPR (BLS) certification At least 6 months recent Nurse Practitioner experience preferred
