Experienced Nurse Practitioners seeking autonomy and a flexible schedule will thrive in this per diem, home health opportunity. In this role, you will conduct comprehensive risk assessments for Medicare Advantage plan members throughout Nueces County, including areas such as Corpus Christi, Agua Dulce, Driscoll, and Port Aransas. Each visit typically takes about 45 minutes and may range from annual health assessments to post-discharge evaluations. Your expertise will support members well-being by identifying care gaps and providing actionable recommendationswithout managing treatment plans or prescribing medications.

Schedule & Flexibility:

Open shifts available Monday through Sunday Set your own schedule with minimum weekly or bi-weekly hour commitment

Key Responsibilities:

Perform annual and post-discharge home health risk assessments Review and evaluate patients prior/current medical and medication history Educate members on medical conditions and medication adherence Identify gaps in care and advise on closing those gaps Visits are documentation-focused; no prescribing or treatment plan development is required

Qualifications:

Active Nurse Practitioner license in Texas Board certification (AANP or ANCC) Current CPR certification (BLS) MSN from an accredited school of nursing Minimum of 6 months NP experience preferred Strong communication and assessment skills

Benefits:
