Key Responsibilities:

Perform comprehensive annual or post-discharge health risk assessments within patients homes Travel to patients throughout Wichita Falls, Burkburnett, Electra, Kamay, and other areas in Wichita County Review prior/current medical history, assess medical conditions, and evaluate medication histories Educate individuals on their health conditions and prescribed medications Identify care gaps and make appropriate recommendations to support better health outcomes

Qualifications:

Active Nurse Practitioner License in Texas Board Certification via AANP or ANCC Graduation with an MSN from an accredited nursing program Current CPR (BLS) certification At least 6 months of nurse practitioner experience preferred In this role, youll earn between $80-$90 per completed evaluation, with additional mileage reimbursement and compensation for administrative duties.
