Shift Information:

The LVN will provide direct patient care within scope of practice, perform follow-up assessments, gather relevant health/social information as delegated by provider, and coordinate care to implement treatment plan.
Required Education:
Graduate of an accredited school of nursing
Required Skills:
Nursing skills as defined in Nursing Standards of practice. Basic computer skills.
Required Licensure/Registration/Certification:
Current Licensed Vocational Nurse in state of Texas; CPR; Texas Driver's License; Valid Auto Liability Insurance. Must have reliable transportation.
Required Experience:
Minimum of two years clinical nursing experience.
Preferred Experience:
Hospice/Palliative or case management experience.
