Responsibilities:

Complete comprehensive home health risk assessments for Medicare Advantage members Review and evaluate prior and current medical history, including conditions and medications Provide patient education regarding health status, medications, and recommended preventive measures Identify care gaps and deliver recommendations for further follow-up or resolution No requirement to develop treatment plans or prescribe medications

Qualifications:

Active and unrestricted Nurse Practitioner license in Texas Current CPR (BLS) certification Board Certification through AANP or

ANCC MSN

degree from an accredited nursing institution Minimum of six months recent Nurse Practitioner experience preferred This position offers compensation of $80$90 per evaluation, along with a mileage stipend and administrative time.
