Life:
Perform comprehensive nursing assessments of patients and families to identify physical, psychosocial, and emotional needs Develop, implement, and evaluate individualized, patient-centered care plans focused on comfort, symptom control, and quality of life Provide skilled nursing interventions, including pain and symptom management, medication administration, wound care, and ongoing clinical monitoring Utilize critical thinking and clinical judgment to recognize changes in condition and intervene appropriately Serve as the primary RN case manager, coordinating care across the interdisciplinary team Collaborate closely with physicians, nurse practitioners, social workers, chaplains, and hospice aides to ensure seamless care delivery Facilitate communication among team members to support timely and effective care planning Maintain accurate, timely, and thorough nursing documentation in accordance with regulatory and organizational standards (Wellsky/Consolo) Participate actively in interdisciplinary team (IDT) meetings, care conferences, and ongoing care planning
Qualifications:
Current Registered Nurse license in the state which you will be working in. One (1) years experience as a RN Case Manager or similar role working for a hospice agency is preferred. Must have a current Driver's license and dependable transportation.
