Job Description

Clinical Education & Onboarding RN ( Home Health & Hospice) The Clinical Education & Onboarding RN is responsible for leading the onboarding, education, training, and ongoing professional development of clinical employees across both Home Health and Hospice services. This position works closely with Human Resources and leadership to ensure new employees receive a consistent, organized, and comprehensive onboarding experience and are fully prepared to perform their roles safely and effectively. The Clinical Education & Onboarding RN will provide ongoing clinical education, competency validation, field support, and individualized training to employees. A key responsibility will be conducting monthly ride-alongs and in-person field education with clinical staff to observe workflows, identify educational opportunities, reinforce best practices, and provide real-time coaching. This position requires an experienced RN with strong knowledge and practical experience in both Home Health and Hospice who can effectively educate staff across multiple disciplines and clinical settings. Key Responsibilities Employee Onboarding & Orientation Partner with Human Resources and leadership to coordinate the clinical portion of new employee onboarding. Develop and maintain a standardized onboarding process for Home Health and Hospice employees. Provide new employees with education regarding company policies, clinical expectations, documentation standards, workflows, and patient-care processes. Ensure new clinical employees understand HCHB and other required systems and workflows. Coordinate clinical orientation and competency requirements based on the employee's position. Track completion of required orientation, education, competencies, and documentation. Identify employees who need additional training or support during their onboarding period. Serve as a clinical resource and point of contact for new employees during their transition into the organization. Ongoing Clinical Education Develop and provide ongoing education for Home Health and Hospice employees. Identify educational needs through chart reviews, audits, staff feedback, clinical trends, and leadership observations. Provide education related to documentation, regulatory requirements, patient care, clinical standards, and company expectations. Develop monthly education topics based on identified needs and organizational priorities. Provide both group education and individualized one-on-one training. Maintain educational materials, competency tools, and training resources. Collaborate with clinical leadership to address recurring documentation or clinical concerns. Monthly Ride-Alongs & Field Training Conduct monthly ride-alongs with clinical employees across Home Health and Hospice. Observe employees in the field and provide hands-on education and coaching. Evaluate clinical practice, documentation habits, patient interaction, safety, and adherence to company standards. Identify strengths and opportunities for improvement. Provide immediate education and feedback when appropriate. Develop individualized education plans for employees requiring additional support. Follow up on previously identified education or competency needs. Document ride-along findings and communicate significant concerns or trends to leadership. Home Health & Hospice Education Provide education specific to both Home Health and Hospice clinical practice. Support clinicians with understanding the differences between Home Health and Hospice workflows, documentation, patient goals, and regulatory expectations. Educate staff on appropriate clinical documentation and care coordination. Provide guidance regarding patient eligibility, clinical processes, interdisciplinary communication, and appropriate escalation of concerns. Serve as a clinical resource for nurses, therapists, aides, and other field staff as appropriate. Competency & Performance Support Develop and maintain competency checklists for clinical positions. Assist leadership with identifying employees who require additional education or competency validation. Provide remediation and individualized training when performance concerns are identified. Partner with managers and clinical leadership to support employee development. Assist with annual competency and education requirements. Maintain accurate records of completed education and competencies. Quality & Regulatory Support Work collaboratively with Quality, Clinical Leadership, HR, and Operations to identify education opportunities based on audits, deficiencies, trends, and regulatory changes. Provide education designed to improve documentation accuracy, clinical quality, patient safety, and regulatory compliance. Assist with preparation for surveys and audits through staff education and competency validation. Monitor recurring clinical or documentation issues and recommend educational solutions. Qualifications Current, unrestricted RN license. Minimum of 3-5 years of clinical nursing experience, preferably with significant experience in both Home Health and Hospice. Strong understanding of Home Health and Hospice clinical workflows and documentation requirements. Previous experience educating, mentoring, precepting, or training clinical staff strongly preferred. Strong understanding of OASIS, Home Health documentation, Hospice documentation, and regulatory expectations. Experience with HCHB or another Home Health/Hospice EMR preferred. Excellent communication and interpersonal skills. Strong organizational and time-management skills. Ability to work independently while collaborating effectively with leadership and multiple departments. Comfortable providing constructive feedback and coaching in a supportive manner. Willingness and ability to travel to patient homes and offices for ride-alongs and in-person training.
