Key Responsibilities:

Complete annual home health risk assessments to support Medicare Advantage plan quality goals Evaluate medical and medication histories and any current or prior health conditions Educate patients on their chronic health conditions and prescribed medications to ensure understanding and adherence Recognize care gaps, making recommendations to optimize patient outcomes Visit locations throughout Llano County as assigned during each shift No treatment plan development or medication prescribing duties required

Qualifications:

Active Nurse Practitioner license in Texas Board certified (AANP or ANCC) Current BLS/CPR certification MSN degree from an accredited nursing program Minimum 6 months of NP experience strongly preferred
