Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Review and document prior and current medical histories, conditions, and medications Educate patients regarding their medical conditions and prescribed medications Identify care gaps and provide recommendations to promote optimal patient outcomes No need to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in Texas Current CPR certification (BLS) Board certification through AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of recent NP experience preferred
