This position involves working closely with patients, families, and healthcare teams to develop and implement comprehensive care plans. The ideal candidate will have a strong background in various medical disciplines and possess excellent communication skills to effectively manage patient needs across different settings. Duties Conduct thorough assessments of patients' health status and needs. Develop, implement, and evaluate individualized care plans based on diagnostic evaluations. Collaborate with multidisciplinary teams to facilitate discharge planning and transitions of care. Provide education and support to patients and their families regarding treatment options and resources. Manage cases involving complex medical conditions, including infectious disease care and geriatrics. Coordinate care in various settings such as hospital medicine, emergency medicine, critical care, and outpatient clinics. Excellent communication and interpersonal skills for effective collaboration with healthcare teams and patient interactions. Strong organizational skills to manage multiple cases efficiently.
