This per diem role focuses on performing thorough evaluations for Medicare Advantage plan members in Marble Falls and the greater Burnet County area. Each visit lasts approximately 45 minutes and may serve as an annual assessment or post-discharge check-in. Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage plan members Review patients past and current medical history, conditions, and medication regimens Provide guidance and education on chronic and acute health conditions, as well as prescribed medications Identify care gaps and offer actionable recommendations No prescribing medications or developing treatment plans required

Qualifications:
