Responsibilities:

Perform annual health risk assessments in patients homes, each visit averaging 45 minutes Evaluate patients medical history, current health conditions, and medication usage Educate patients about their medical diagnoses and prescribed treatments Identify gaps in care and collaborate with healthcare teams for resolution No treatment plan development or medication prescribing required

Qualifications:

Active Nurse Practitioner license in Texas Board certification via AANP or

ANCC MSN

degree from a recognized institution Current CPR (BLS) certification At least six months of recent NP experience is preferred This position offers competitive earnings of $80$90 per evaluation in addition to mileage reimbursement and compensation for administrative tasks, making it a highly rewarding and flexible per diem role.
