Job Description
Aveanna Healthcare is seeking a compassionate Licensed Vocational Nurse to provide one-on-one adult skilled nursing care in the home and community. In this role, you'll deliver direct patient care, including assessments, medication administration, wound care, and monitoring of chronic conditions. You will follow individualized care plans, document accurately, and collaborate with an interdisciplinary team to support optimal outcomes. Responsibilities Provide one-on-one skilled nursing care to adult patients in the home and community Perform comprehensive assessments, monitor vital signs, and track changes in condition Administer medications and treatments per physician orders and scope of practice Deliver wound care, chronic disease management, and other skilled interventions Implement and follow individualized care plans and clinical protocols Document care accurately in electronic health records and maintain required reports Communicate with physicians, therapists, and care team to coordinate services Educate patients and families on disease management, safety, and care procedures Adhere to infection control, safety, and regulatory standards Participate in ongoing training and competency development offered by Aveanna Required Skills Adult patient assessment Medication administration Wound care Care planning and documentation IV therapy (where permitted) Ventilator and tracheostomy care Infection control Electronic health records (EHR) Patient and family education Time management
