Each assessment averages 45 minutes, covering annual health evaluations or post-discharge visits as needed.

Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage plan members Review patient medical history, current health conditions, and medications Offer education regarding medical conditions and medication management Spot and address care gaps, providing actionable recommendations Focus solely on assessmentsno treatment plan development or prescribing required

Qualifications:

Active Nurse Practitioner license in Texas Board certification via AANP or ANCC Current CPR (BLS) certification MSN degree from an accredited nursing program At least 6 months of experience as a Nurse Practitioner is preferred
