Connect Pediatrics seeks compassionate Home Health Nurses (RN or LVN) to provide one-on-one skilled nursing to medically fragile children in their homes. You will deliver pediatric assessments, medication administration, trach/vent and G-tube care, and ongoing monitoring while following individualized care plans. Collaborating with families, physicians, and an interdisciplinary team, you'll support safe, developmentally appropriate care and educate caregivers. Responsibilities Provide skilled nursing care to pediatric patients in their homes (RN or LVN scope). Assess, monitor, and document vital signs, symptoms, and response to treatment. Administer medications, enteral feedings, respiratory treatments, and other ordered therapies. Execute and update individualized care plans in collaboration with physicians and families. Educate and support families on disease management, equipment, and home care techniques. Use and manage pediatric medical equipment such as trachs, vents, and feeding tubes as assigned. Communicate changes in condition promptly to the care team and participate in case conferences. Maintain accurate, timely clinical documentation in EMR per agency and regulatory standards. Follow infection control and safety protocols to ensure a safe home environment. Participate in ongoing training and continuing education to maintain clinical competence.
