Home Health Nurse- RN/LVN Case Managers

Licensed Vocational Nurses (LVNs) Registered Nurses (RNs)

Employment Type:

Part-Time / PRN to Start As our patient census continues to grow, these positions have the opportunity to transition into full-time roles.

Compensation:

Pay Per Visit The more patients you see, the more you earn.

Requirements:

Current Texas nursing license (LVN or RN) Minimum 1 year nursing experience required Home health/home care experience preferred Reliable transportation and valid driver's license Strong assessment, documentation, and communication skills

Responsibilities:

Perform skilled nursing visits in patients' homes Complete timely and accurate documentation Educate patients and caregivers Coordinate care with physicians and interdisciplinary team members Monitor patient condition and report changes as needed
