In this role, you will provide high-quality patient care to individuals in hospice settings at their residence, ensuring comfort and dignity during end-of-life transitions. The ideal candidate will possess strong clinical skills, a deep understanding of patient needs, and the ability to work collaboratively with families and interdisciplinary teams. Duties Deliver comprehensive nursing care to hospice patients in various settings, including home health and nursing homes. Conduct thorough patient assessments to evaluate physical, emotional, and spiritual needs. Develop and implement individualized care plans in collaboration with healthcare teams and family members. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Provide education and support to patients and their families regarding disease processes, symptom management, and end-of-life care options. Assist with behavioral health considerations as part of holistic patient care. Document patient progress accurately using electronic health record systems such as Epic. Participate in interdisciplinary team meetings to discuss patient care strategies and outcomes. Maintain compliance with all regulatory requirements and best practices in hospice care. Requirements Valid nursing license (RN ) in the state of Texas. Experience in working within a hospice environment is preferred. Familiarity with hospital medicine practices, particularly within a Level I Trauma Center setting. Excellent communication skills for effective interaction with patients, families, and healthcare professionals. Ability to work independently while managing multiple priorities in a fast-paced environment. Compassionate demeanor with a commitment to providing quality end-of-life care.
